A quality report card, within the context of health and wellness, is a report generated by a third-party evaluator that rates the performance of a specific health care provider. A quality report card may also be referred to as a health care quality report card.

A quality report card reports the results of an independent review of a health care provider's service and performance. The organization preparing the quality report card will evaluate the healthcare institution based on a predetermined set of criteria including specific quality standards and performance measures. Quality report cards may be prepared for individual health care providers or large health care organizations. The U.S. Agency for Healthcare Research and Quality (AHRQ) assembles health care reports from multiple providers in a searchable collection, the Report Card Compendium, to assist consumers in reviewing the quality of their health care provider.
